Pelvic organ prolapse occurs when the muscles and tissues supporting the pelvic organs (the uterus, bladder, or rectum) become weak or loose. Under typical conditions in women, the bladder is held up by a “hammock” of supportive pelvic floor muscles and tissue. The weakening or looseness allows one or more of the pelvic organs to bulge out of the vagina or drop.

The different types of pelvic organ prolapse depend on the pelvic organ affected. The most common types include:

  • Cystocele and Urethrocele – Loss of bladder support. A bulging of the upper front vaginal wall where a part of the bladder bulges into the vagina. Also known as a prolapsed bladder, it may result in women experiencing urinary incontinence (loss of urine), frequency, urgency, and retention.
  • Rectocele – A bulging of the upper rear vaginal wall where a small bowel portion bulges into the vagina. Women may notice the bulge when standing leads to a pulling sensation and backache that’s relieved when lying down.
  • Enterocele – Loss of upper vagina support and loss of vaginal sidewall support
  • Uterine prolapse – When the uterus drops down into the vagina

Diagnosis and Treatment

During a pelvic exam, your doctor will discuss your pelvic symptoms and may complete other tests to see whether you can completely empty your bladder when you go to the restroom. You may also be asked to cough or strain during the exam so your doctor can see if those actions cause any bladder leakage or prolapse.

Treatment varies for each woman, depending on the type of prolapse present, age, symptoms, and any other health problems. Surgeries for each type of prolapse is available:

  • Hysteropexy – Uterine prolapse surgery
  • Sacrocolpopexy – Vaginal vault prolapse surgery
  • Posterior Repair – Procedure to fix the problem of rectocele
  • Paravaginal Repair – Procedure to fix the problem of cystocele

Your treatment may also include:

  • Pessary
  • Pelvic floor muscle therapy
  • Changing eating habits
